Use Treatment of pulmonary arterial hypertension in patients next to NYHA Class III or IV symptoms.
Mechanism of Action-Acutely, iloprost dilates systemic and pulmonary arterial vascular beds. In longer-term use, alters pulmonary vascular resistance and suppresses vascular smooth muscle proliferation. In enhancement, it is a potent endogenous inhibitor of platelet aggregation.
Pharmacodynamics/Kinetics
Duration: 30-90 minutes
Metabolism: Hepatic via beta oxidation of the carboxyl side chain; key metabolite, tetranor-iloprost (inactive in animal studies)
Half-life removal: 20-30 minutes
CAUTIONS : The most common adverse effects of inhaled iloprost are flushing (vasodilatation), increased cough, headache, and trismus (lockjaw). Serious events include pulmonary edema, which is a sign of pulmonary venous hypertension. Iloprost should not be initiated contained by patients with systolic blood pressure smaller amount than 85 mmHg because of the risk of syncope. Other adverse effects include insomnia, nausea, and vomiting.
Iloprost is a more stable synthetic analogue of prostacyclin (epoprostenol). It is a comparable pulmonary vasodilator to prostacyclin and therefore have potential for the treatment of pulmonary hypertension. But I'm seeing in studies on everything from HIV to Diabetes.
