and that woman had to pocket the needle out

Answers: The vein infiltrated. This happen alot, does not depend on how healthy you are or anything, what happen is when blood is coming throught he vein does shake for a time and probably what happened what the nozzle caught the other side of the vein and cause it to collapse therefore bleeding around the holes contained by the vein. No biggie.
The bevel or tip of the plunger could have be displaced. Most commonly it had pierced and perforate the vein and the syringe tip got out of the artery. Some of the venous blood had after extravasated or went out of the artery to cause the swelling. Nothing to verbs.
