As long as he graduated, he will be call a newly graduate doctor. After graduation he will go through internship, so he will call a medical doctor "intern". Going to the next step he will hold the residency training so he will be called a resident doctor.

might be rock-hard for him to find residency. but beyond that, it doesn't matter. school over. nobody will ever look at your grades again.
In my year, they would have elected him president of Zeta Omega Zeta, the facetious divergent of AOA the fraternity of the top ten percent of medical schools. Some the best people-friendly doctors are not glorious achievers. (Any US grad would hold little trouble getting a residency slot.)
