Give an account of the viruses which cause severe respiratory tract infections in humans, describing the epidemiology, transmission, pathogenesis and control, with particular reference to influenza.

Influenza is a regularly occurring epidemic which is a major cause of world wide morbidity and mortality. It causes a range of clinical conditions that affect all parts of the respiratory tract.

Influenza A affects a variety of animals and birds (and humans), Influenza B infects only humans.

PATHOGENESIS AND CLINICAL FEATURES. The virus enters the respiratory tract via air born droplets and attaches to sialic receptors on epithelial cells. 3 days after this symptoms will appear such as fever, malaise, chills and muscle pain. As the virus develops further, symptoms of runny nose, sore throat and dry cough are experienced. Recovery is 1-3 weeks, but may be prolonged with serious infections developing into pneumonia, or bronchitis.


TREATMENT & PREVENTION: Best treated with rest, analgesia and fluids. Antibiotics are only effective when there is a secondary infection. Prophylactic drugs to limit infection in the vulnerable elderly age group include Amantadine and rimantadine. Influenza vaccine is good for partial protection before the influenza season (winter). Recommended for health care workers, the elderly & patients with chronic disease.
