I would suggest you complete all steps of the USMLE beforehand coming to the U.S. Each residency program will have their own set of rules on what they require for entry to their programs so you may wish to start contacting individual programs to find out what they may be (I enjoy had both U.S. med students and foreign med students start contacting me already). I can also recount you that while I don't know what specialty you plan to go into, if you do your residency contained by spain and then conceivably choose to come to the U.S. to do a fellowship, you may run into difficult in that lots program directors prefer someone with experience surrounded by U.S. hospitals.
