Undergraduate college - 4 years (this is after high school)
Medical School (called medical students) - 4 years
Residency (first year call intern, residents thereafter) - 3-8 years
Fellowship (called fellows, optional) - 1-3 years
Attending - years after that

College student -> medical student -> intern -> resident -> fellow -> attending

BTW, a cardiologist cannot become a cardiac surgeon. The pathway to a cardiothoracic surgeon is through broad surgery and then a fellowship within cardiothoracic surgery.
