Answers: Vacutainer tubes are hermetic with adjectives gas expelled--that is they have a vacuum inside (when you pop stale the top that pop is air rushing in). So, they suck blood through the plunger. When the vacuum is gone (no more "suck") the tube is filled. The flange is the little manipulate wings on the vacutainer holder (the washed out plastic thing). You hold onto the holder & push the vaculatiner tube forward onto the needle explicitly inside it that punctures the cap of the tube and allows you to draw blood. This will be clear the minute you see a draw done.
