You assess a patient as soon as you congregate him. We do a head to toe assessment at least possible once per shift. Then you monitor them for changes contained by their condition or changes within your assessment of them.

When she monitors a patient, she is collecting information to determine changes within the patient's condition over time.
Monitoring is "expermet and obseration" A monitor, and the eyes and ears of a medico are used. . Assesment is "inference" and the medico`s brain is used.
