Jaw exhaustion give a hand!?

i watch a video on youtube and i swot up that there are two main types of surgery to reduce cheek, we have chin botox surgery and bone jaw surgery, cheek botox is easy and rationalized because the doctor just remove muscles on jowl part, while on bone cheek surgery, they cut bone that's why it appears to be a jaw of animal because it is obiously cut... how can a character know it he/she needs botox chin surgery or bone jaw surgery?? what are the different factor that let you know weather he/she have a muscular jaw or particularly bony jaw?? is it true that while you are turning elder your jaw grows too??
