To be honest, this procedure is very seldom (if ever) covered by traditional insurance companies. It's considered an elective procedure--- classified the same style as plastic surgery. And, since all state law require people to be counseled something like the permanence of sterilization (for both men and women) the insurance companies own no legal cause (and to be honest, no financial reason) to cover it.
The success rates are also suspect.
Many insurance companies will rate for some basic infertility services (like testing), but not within vitro fertilization for the same principle. But, you might want to consider egg harvesting and assisted reproduction instead of the reversal.

You should ring up them and ask, your card should have an 800 number on the support where you can send for. All insurance companies have abundantly of different policies so even if I told you my Horizon Mercy policy covered it that doesn't mean yours will.
