How do I go about getting a copy of my medical records?
Answer:
You request them from your doctor..and get this, they CHARGE you for it! And it's all about you.what gives? It stinks I know but that's just how it goes. If there is a way around this, I would love to know.so I'm looking forward to seeing some of the other answers as well.
make a request at your surgery you may need some interpretation of terms. If you want to look at hospital records you may have to pay a fee.
call your doctor's office make a request. If you are going to pick them up tell them you will sign the authorization papers when you get there. If you want to have them mailed , you may have to have the written request faxed or mailed to you. Sometimes they will do it by verifying your S.S.# and date of birth over the phone.
sign a request at the office and wait till they copy it. Or have your new doctor send for it.
Contact your Physician and ask for the copy you need. They are generally done by the Office staff, some Physicians will have a charge for this service and others do it as a courtesy. Hope this is helpful
If they are from a hospital in the UK then contact the Medico-Legal Department. If you have been treated at the hospital within 40 days of your requesting to view the notes then there is no charge.

tell your GP you want all your medical notes to date. You are by law aloud to have your own copy. Even if he passed any of your medical problems to another doctor at a hospital, its down to your GP to track all the records down.
