If they were licsenced they'd most likely no more about it than your doctor. You should see a doctor anyway to get treated for the infection though.

However, it'd probably hurt less if you did it yourself. Try leaving it in and using peroxide on it. Put some neosporin or another antibiotic ointment on it, too. If you must take it out while it's that swollen, try icing it a few times. Use ice for 30 seconds at a time, then stop icing it for 30 seconds. Once you do it enough times to get it numb, quickly and swiftly pull it out.
Dont take your piercing out! If you do the infection could spread, at this point you need to keep it clean and take care of it as much as possible. Again DONT TAKE IT OUT!
if it is downstairs, you better get to an emegency room asap. if it is just you ear or nose, try repeatedly using hot soapy water and keep washing and rinsing. if that doesn't turn the tide, rush to the emergency rooms because you could have it spreading all over you body in a hurry. like in your brain and your heart or kidneys. any of that could kill you. no joke.
That's needs a doctor's attention. He or she will be able to remove it but I'm sure the pain will still be present unless if the pain is that excruciating he or she may want to give you some pain killers
