Not common at his age. I'd imagine recovery is going to be a hideous thing at his age, since skin (and just in general, the body) reheals much slower in senior citizens.
Is it needed? Did he get an infection? If he just got an infection, tell him to ask for antibiotics and to keep it clean by pulling it back and rubbing the head when he showers. It only takes five to ten seconds to do. That'll prevent any infections. Chances are that doctors just suggest surgery rather than simply antibiotics because it brings a lot more money to them.
