Extensive-stage disease is cancer outside a single hemithorax and presence of malignant pleural or pericardial effusion. About 1⁄3 of patients with SCLC have limited-stage disease; the remainder often have extensive distant metastases.

Well, I don't know much about lung cancer, but I would have to say the last stage is the worst.

Stage 4 is the worst. My husband was originally diagnosed with stage 4 because it went to his brain before they found it. He had stage 4 non small cell lung cancer.He survived 3 years but lost the battle when it took over his body.To me the worst was not so much the stage 4 but the effects it had on his body at the end. He could not function.
Stage four. It has spread through out the body. It is HORRIBLE!!!
