Why would very healthy 18 year old female housecat start pulling hair out of chest and legs ?

Stress. I had a cat who was a bit younger than yours and he did the same thing whenever my sister was stressed out. Examine your situation at home... are you or anyone living with you stressed out? Cats pick this up b/c the are very sensitive animals. Find the root of the problem and try to work on that. Whatever the situation, when it dies down, your cat will stop pulling its hair out.
Sexual frustration.
allergy, some type of neurosis, or thyroid problem (only if hair falls out by itself). take her to the vet!
feline dementia
She's real itchy and has fleas.

could be an allergy. when cats get to that age, their immune system does start to go, no matter how healthy they are. they start to get allergies and things like that.
take her to the vet for a complete checkup. im not sure this could be something you could change yourself without some professional advice.
Could be anything to a sudden allergy to stress to an illness/pain by where she is pulling out the fur... to all manner of things. Take her to a vet, please :-)
