In some cases the teeth need to be surgically removed because they are impacted within bone and tissue. General anesthesia is recommended in complex extractions such as those.
I have all four of mine removed below local. They were impacted, and I'd consider it a pretty rough article to go through widespread awake. Not so much the pain really, it's the dude trouncing them apart with the little chisel and tack hammer with his unbroken hand surrounded by your mouth and the taste of blood that probably bugged me the most. I be the only one he'd ever done below local and he was plainly nervous something like it. I didn't have insurance at the time and have to cut corners wherever possible. I feel of your insurance will cover general, afterwards just hold it done that way. Many dentists will merely do it under common anesthesia.
PS: Nothing bothers me.. I broke my thumb at work today and finished my shift at the ole warehouse. Having my wisdom teeth out close to that even bothered me a bit.
